Wellness Incentive

Partners who complete an annual wellness exam with a primary care provider between December 1, 2025, and November 30, 2026, are eligible for a Schreiber-funded Wellness Incentive.

Eligibility

Benefit-eligible partners enrolled in a Schreiber medical plan qualify for the wellness incentive. Spouses are not eligible.

Incentive

Once your wellness exam is submitted in Workday, your incentive will be processed and paid within two pay periods.

  • Surest Plan Participants: You’ll receive a $300 incentive, paid directly on your payslip and taxed as supplemental income.
  • High Deductible Health Plan (HDHP) Participants: If you’re enrolled in the HDHP and have an active Health Savings Account (HSA), your incentive will be deposited into your HSA:
      • $600 for employee-only coverage
      • $1,200 for all other coverage tiers 

Important Reminder: To receive the HSA funding, HDHP participants must be enrolled in the HSA. You will not receive the incentive if you are not enrolled. To enroll in the HSA, view the instructions here

Earning the Incentive

To qualify for the 2026 wellness incentive, partners must complete an annual wellness exam with a primary care provider between December 1, 2025, and November 30, 2026.

To receive the incentive:

  • Submit either the Annual Wellness Exam Form or other valid proof of the exam (such as an After Visit Summary with the date and visit reason showing) in Workday by November 30, 2026.

Instructions to Submit Documents

  1. Log in to Workday and click on the Menu in the upper left corner.
  2. Select Personal Information, then click Worker Documents (in the View column on the right).
  3. Click Add, then Select files and choose the documentation you want to upload, then click Open.
  4. Choose Benefits – Annual Wellness Exam.
  5. To load another document immediately, click upload. If you are done uploading documents click OK, and then click Done.
  6. If you need to add more documents, begin again from step number 3.
