Lifestyle Spending Account (LSA)
This account allows users to allocate funds towards various lifestyle expenses that contribute to their physical, mental, and emotional health. Whether it’s fitness memberships, fishing equipment, or personal development courses, the LSA empowers partners to invest in activities that align with their individual needs and preferences that promote their well-being. Partners can be reimbursed up to $600 for eligible items purchased and submitted in Forma between January 1st, 2026 and November 30, 2026. In addition to the reimbursement option, partners can use the Forma marketplace to purchase eligible items directly through the portal with the available funds, no reimbursement needed!
Eligibility
Benefit-eligible partners are able to access the $600 benefit between January 1st, 2025 and November 30, 2025. Partners may submit eligible expenses incurred between December 1, 2025, and November 30, 2026. All approved expenses will be applied against the 2026 benefit allocation. December 2025 expenses can be submitted starting January 1, 2026.
Note: Partners must be active at time of payout to be eligible for reimbursement.
Important: To receive reimbursement, you must be an active partner at the time of payout. Please review the payout calendar carefully to know when to submit your documents in Forma, ensuring eligibility before retirement or termination.
First Time Registration Instructions
Step 1: To register in Forma, go to https://client.joinforma.com/schreiber_foods/activate.
Important: Do not download the Forma app until you register online through the above Forma registration link.
Step 2: Fill in the required details and click submit details.
- Last name exactly as it’s written in Workday (include special characters and both last names if more than one)
- Date of Birth
- 6-digit Workday ID (can be found on the top portion of your payslip in Workday or in your Workday profile)
Step 3: Enter your preferred email address and click Activate via Email.
- Make sure you enter an email address you will have access to when logging into the portal. If you enter the incorrect email address, contact Forma support at support@joinforma.com for assistance.
Step 4: Check your email inbox and click Log in to Forma, then click Get Started. Review the benefit offering and click Continue.
Step 5: Create a password, re-enter your email address, agree to the terms, and click Activate my account to complete the process.
Step 6: Once your account is activated, you can submit your claims for reimbursement. Ensure that your items are on the approved list and were purchased between 12/1/25-11/30/26. Once your account is activated, you can submit your receipts directly through the Forma website or in the Forma app.

Reimbursement Instructions
To receive reimbursement for eligible items purchased through the Lifestyle Spending Account (LSA), follow these steps:
Step1: Go to Forma and enter the email address you used to register and click Get Magic Link.
Important: If you do not receive the link to your email, try another email you may have used to register. If you no longer have access to that email address, contact Forma support for assistance (support@joinforma.com).
Step 2: Go to your email account and open the email from Forma. Click the Log In to Forma button.
Submit Receipts: Upload your receipts between January 1st, 2026 and November 30th, 2026.
Reimbursement Process: Ensure that the receipts clearly display the date of purchase, the item purchased, and the purchase amount. Handwritten receipts will not be accepted. Once your claim is submitted and approved, the reimbursement will be processed and included in your bi-weekly payslip, and taxed as supplemental income.
Direct Purchases: You can also use the Forma marketplace to purchase eligible items directly through the portal with available funds, eliminating the need for reimbursement.
LSA Balance: When you log into Forma, you’ll see your remaining balance for the current calendar year.
Reimbursement Schedule
Reimbursements will be included in your bi-weekly payslip and taxed as supplemental income. Depending on when your claim is approved, you will receive your reimbursement within 2-4 weeks. Please see below for estimated payout dates based on when your item is approved in Forma.
