Carrot provides comprehensive hormonal health, fertility, and family-building benefits. Whether you’re interested in preserving eggs, sperm, or embryos for the future, or are currently pursuing fertility treatments to expand your family, Carrot is here to help. Through Schreiber’s partnership with Carrot, eligible employees and their spouses can access up to$20,000 in employer-sponsored funds to help cover approved fertility services.
Benefit Details
With Carrot, your fertility treatment options may include:
- Fertility testing: At-home or in-clinic options to better understand fertility health, including hormone levels, ovarian reserve, and sperm quality
- Ovarian stimulation and/or ovulation induction: Medications to help the ovaries produce mature eggs and/or encourage the body to release an egg at the right time
- In vitro fertilization (IVF): A procedure where eggs are retrieved from the ovaries and fertilized by sperm in a lab to create embryos, which may be transferred right away or frozen for future use
- Insemination: Treatments like intrauterine insemination (IUI), where a prepared semen sample is placed inside the uterus around ovulation to support fertilization
- Embryo transfer: Placement of either a fresh or frozen embryo into the uterus
- Cryopreservation: Freezing and storing eggs, sperm, or embryos for future use

Financial Benefit
Accessing the Financial Benefit
To access the financial benefit, members must first meet their annual deductible. Once the deductible is met, they can choose to either pay upfront and request reimbursement from Carrot or use the Carrot Card to cover eligible expenses directly—no reimbursement required.
High-Deductible Health Plans (HDHP)
Before you can access the financial benefit, you must meet your medical plan deductible.
Carrot keeps track of your deductible status in real time. If you haven’t spent anything on medical care yet this year, you’ll need to pay for fertility services out of pocket until you reach your deductible.
Even though you’re paying upfront, Carrot will send those expenses to UMR, so they count toward your deductible and your yearly out-of-pocket maximum.
Once you’ve met your deductible, you’ll unlock Carrot’s financial support. At that point, you can either:
Request reimbursement for expenses incurred after meeting your deductible.
Use the Carrot Card to pay directly for expenses incurred after meeting deductible — no reimbursement needed.
Surest Copay Plan
Before you can access the financial benefit, you’ll need to meet a $1000 Carrot-specific deductible—this is a set amount you must pay out of pocket for fertility care before Carrot’s financial support kicks in.
Until you reach that amount, you’ll pay for fertility services yourself. Carrot will send those payments to Surest, so they count toward your annual out-of-pocket maximum.
Once you’ve met the Carrot-specific deductible, you’ll have two options:
Request reimbursement for expenses incurred after meeting your deductible.
Use the Carrot Card to pay directly for expenses incurred after meeting deductible — no reimbursement needed.
